Required athletic participation forms for the 2023-24 school year is now available. Registration can be done online at the Family ID website. Access to these forms are available on this site by clicking on the more tab and then the files and links tab. You can then download and print the following required forms: Physical Exam, Athletic Handbook.The Insurance form is also available but it is also included in the Athletic Handbook packet. Three(3) forms is required to be turned in prior to the 1st practice:

1. Physical Exam/Parental Consent form. Signed by parent AND student

2. Handbook Acceptance/Drug Testing Consent Form. Signed by Parent AND Student

3. Insurance Coverage Form

If you do not have access to a printer please call the school as these forms are available in the main office or from the Athletic Director.

All Forms can be downloaded by clicking on the "More" tab at the top right of home page and then click on "Files and Links". Once completed, these forms can be uploaded on the Family ID site for submission or print and give to Athletic Director or Head Coach. These forms must be turned before the 1st practice to participate.
